今回は、前立腺がんの腫瘍マーカーとして知られるPSAについてまとめました。ある年代になると、男性の同窓会でお互いのPSA値を話題にする、という話をよく耳にします。

◆ PSA
PSAはProstate Specific Antigenの頭文字をとったもので、日本語では「前立腺特異抗原」と呼ばれます。前立腺がんの腫瘍マーカーとして広く知られており、自治体のがん検診にも含まれるため、多くの方に馴染みがあるかもしれません。PSAは採血で測定でき、一般的には4.0ng/mL以上が異常値とされます。PSAは前立腺の細胞から分泌されるタンパク質で、ゲル状の精液をさらさらにする役割があるとされています。ただし重要なのは、PSAは前立腺に特有のタンパク質であっても、前立腺「がん」に特有のタンパク質ではないという点です。そのため、PSA値が高くても、必ずしも前立腺がんがあるとは限りません。

 

 

◆ そもそも前立腺とは
前立腺は男性にのみ存在する臓器で、膀胱のすぐ下に位置します。くるみほどの大きさで、精液の一部を作る役割を担い、男性の生殖機能において重要な働きをしています。また、尿道を取り囲むように存在するため、排尿にも大きな影響を及ぼします。前立腺がんだけでなく、前立腺肥大症や前立腺炎などの疾患でも、前立腺の正常な組織構造が破壊されることで血中にPSAが漏れ出し、PSA値が上昇することがあります。前立腺肥大症は加齢による良性の変化であり、前立腺がんとは明確に区別されます。肥大した前立腺により尿道が狭くなるため、頻尿や尿の勢いの低下がみられることがあります。一方、前立腺炎は尿路の細菌感染症で、急な頻尿、排尿時の痛み、高熱が特徴です。適切な抗菌薬治療で治癒します。

 

◆ PSAが高いと言われたら
PSA高値を指摘されたときには、早めに泌尿器科にご相談ください。過去にPSAを測定したことがある方は、そのデータを持参いただけると非常に参考になります。過去のPSAが落ち着いていたにもかかわらず、急に極端に上昇したときは前立腺の一時的な炎症を疑います。超音波検査で前立腺のサイズを測定し、PSA値の推移も考慮した上で、総合的に前立腺がんが疑わしければMRIを撮ります。MRIでも前立腺がんを示唆する所見がある場合は、診断を確定するために前立腺生検を行います。

 

 

当院では会陰部（肛門と陰嚢の間）と前立腺周囲の局所麻酔および仙骨硬膜外ブロックにより十分な鎮痛下で生検を行います（日帰り生検の場合、局所麻酔のみの病院が多いです）。検査は30分程度で完了し、生検後すこしお休みいただき排尿が問題なくできればご帰宅いただけます。前立腺がんは初期段階では自覚症状がほとんどないため、早期発見にはPSAの測定が不可欠です。早期発見された前立腺がんは、根治を目指した治療が可能です。また、悪性度が低く腫瘍が小さい場合は、治療を急がず経過観察が選択されることもあります。ガイドラインでは、50歳以上の男性や前立腺がんの家族歴がある男性に対し、定期的なPSA測定を推奨しています。

 

◆ 検査費用
全く症状もなくPSAの測定のみを希望される方は、検診扱いになりますので以下の自費診療になります。

 

高感度PSA（自費）：2,000円（当院は川口市の検診指定医療機関ではありませんので公費補助は使えません）
あわせて自費初診料 3,000円、自費再診料 1,000円

 

ただし、PSA高値が指摘された場合、それ以降の精査は保険診療となります。他院でのPSA測定で異常値があった場合も、当院での精査は保険診療が適用されます。

 

◆ AGA治療との関係
最近さかんに広告が出されている男性型脱毛症（AGA）の治療ですが、代表薬であるフィナステリド（商品名プロペシア）やデュタステリド（商品名ザガーロ）にはPSAを低下させる作用があります（数値が半減する可能性があります）。これらの薬剤によりPSAが低下すると、前立腺がんのリスク評価が適切にできなくなる可能性があります。AGAの治療を開始する前に、一度PSAをチェックすることをお勧めします。すでにAGAの内服治療を行っている方は、その旨を必ず医師にお伝えください。

This time, we have summarized information about PSA, a tumor marker known for prostate cancer. At a certain age, it’s often said that men discuss and compare their PSA levels at reunions.

 

◆ PSA
PSA stands for Prostate-Specific Antigen. It is widely known as a tumor marker for prostate cancer and is often included in cancer screenings conducted by municipalities, so it may be familiar to many people. PSA can be measured through a blood test, and values of 4.0 ng/mL or higher are generally considered abnormal. PSA is a protein secreted by prostate cells and is believed to play a role in liquefying semen. However, it is important to note that although PSA is a protein specific to the prostate, it is not exclusive to prostate cancer. Therefore, even if PSA levels are elevated, it does not necessarily mean prostate cancer is present.

 

 

◆ What is the Prostate?
The prostate is an organ found only in men and is located just below the bladder. About the size of a walnut, this organ produces part of the seminal fluid and plays an essential role in male reproductive function. Additionally, because the prostate surrounds the urethra, it also has a significant impact on urination.

 

Conditions such as benign prostatic hyperplasia (BPH) or prostatitis, in addition to prostate cancer, can damage the normal tissue structure of the prostate, causing PSA to leak into the bloodstream and increasing PSA levels. BPH is a benign change associated with aging and is distinctly different from prostate cancer. An enlarged prostate can narrow the urethra, leading to symptoms such as frequent urination or reduced urine flow. On the other hand, prostatitis is a bacterial infection of the urinary tract characterized by sudden frequent urination, pain during urination, and high fever. It can be cured with appropriate antibiotic treatment.

 

◆ What to Do If You Have High PSA Levels
If you are told your PSA levels are high, we recommend consulting a urologist as soon as possible. If you have previously had your PSA levels measured, bringing that data with you can be very helpful. When PSA levels that were previously stable suddenly rise significantly, temporary inflammation of the prostate may be suspected.

 

We measure the size of the prostate using ultrasound and evaluate PSA trends to comprehensively assess the likelihood of prostate cancer. If prostate cancer is suspected, we may perform an MRI scan. If the MRI findings also suggest prostate cancer, a prostate biopsy will be conducted to confirm the diagnosis.

 

 

At our clinic, we perform biopsies under adequate pain control using local anesthesia for the perineum (the area between the anus and scrotum) and around the prostate, as well as sacral epidural block if needed. Many hospitals perform same-day biopsies using only local anesthesia. The procedure takes about 30 minutes, and after a brief observation period to ensure there are no urination issues, you can go home the same day.

 

Prostate cancer usually does not present noticeable symptoms in its early stages, making PSA testing essential for early detection. Prostate cancer discovered early can be treated with curative intent. Furthermore, for low-grade cancers with minimal tumor volume, treatment may not be immediately necessary, and monitoring may be an option. Guidelines recommend regular PSA testing for men over 50 years old and those with a family history of prostate cancer.

 

◆ Test Costs
If you have no symptoms and wish to measure PSA levels only, it will be considered a screening test and subject to the following private medical fees:

 

High-Sensitivity PSA: JPY 2,000
Note: Our clinic is not a designated cancer screening facility in Kawaguchi City, so public subsidies cannot be applied.

 

Initial consultation fee: JPY 3,000
Follow-up consultation fee: JPY 1,000

 

If high PSA levels are detected during the screening, subsequent tests will be covered by insurance. Similarly, if abnormal PSA results from tests conducted at other clinics are brought to us, further evaluations at our clinic will also be covered by insurance.

 

◆ Relation to AGA Treatment
Medications for androgenetic alopecia (AGA), such as finasteride (brand name Propecia) and dutasteride (brand name Zagallo), which are widely advertised, have the effect of lowering PSA levels (potentially halving the values). When PSA levels are artificially reduced by these drugs, accurate prostate cancer risk assessment may become difficult. We recommend checking PSA levels before starting AGA treatment.
